ABSTRACT

OBJECTIVE To investigate the efficacy and safety of Shenxiang suhe pill in the adjuvant treatment of triple vessel lesion of coronary artery. METHODS From February 2019 to February 2021,146 patients with unstable angina pectoris and triple vessel lesion of coronary artery (chest discomfort of hanning xinmai type )admitted to Harrison International Peace Hospital were divided into observation group and control group according to random number table ,with 73 cases in each group. Control group was given Isosorbide dinitrate tablets+Aspirin enteric-coated tablets+Clopidogrel bisulfate tablets+Atorvastatin calcium tablets+ Bisoprolol fumarate tablets for western medicine treatment. Observation group was additionally given Shenxiang suhe pill 0.7 g/time orally after meal ,twice a day ,on the basis of control group. After 2 courses of treatment (3 weeks as a course of treatment ,with an interval of 1 week for each course of treatment ),curative effect and symptoms of angina pectoris [frequency and duration of angina pectoris ,walking distance of 6-min walking test (6MWT)],TCM syndrome score ,cardiac markers [neutrophil to lymphocyte ratio (NLR)and the contents of endothelin-1(ET-1),nitric oxide (NO),soluble suppression of tumorigenicity 2 (sST2),angiotensin Ⅱ(Ang-Ⅱ)] and quality of life [Seattle angina questionnaire (SAQ),36-item short form health survey (SF-36)] before and after treatment were compared between the two groups. Adverse drug reactions (ADR)during treatment and major adverse cardiovascular events (MACE)during follow-up were recorded. RESULTS The total response rate of the observation group was 86.30%,significantly higher than 71.23% of the control group (P<0.05). Before treatment ,there was no statistical difference in the above indexes between 2 groups(P>0.05). After treatment ,the frequency and duration of angina pectoris ,TCM syndrome score ,NLR and the contents of sST 2,ET-1 and Ang- Ⅱ in the two groups decreased or shortened significantly ,while the walking distance of 6MWT,NO content ,scores of each dimension in SAQ and SF- 36 increased significantly ;and the above indexes in observation group were significantly better than those in control group (P<0.05). There was no significant difference in the inciden ce of ADR between the two groups (P>0.05); however,the incidence of MACE in observation group was significantly lower than that in control group (P<0.05). CONCLUSIONS In the treatment of adjuvant treatment of triple vessel lesion of coronary artery ,Shenxiang suhe pill can improve the symptoms of angina pectoris and vascular endothelial function ,reduce the incidence of MACE ,improve the quality of life ,and has good safety.

ABSTRACT

AIMS: Cross sectional observational comparative study in a tertiary care hospital on Relation between Ischemic Heart Disease among Physically Active Tribal's of Jharkhand presenting with chest discomfort. MATERIALAND METHODS: 102 physically active Tribal subjects (53 male and 49 female) were selected for the study, who had presented with chest discomfort. RESULTS:The incidence of I.H.D. is 1.41% among tribal population of Jharkhand. The incidence was found exclusively in tribal female living in urban area mostly due to adopting sedentary pattern of life style. CONCLUSION:The tribal population of Jharkhand has inversely proportional relation to Ischemic Heart Disease and physical activity.

Therefore, this paper aims to review the literatures on TCM diagnosis and treatment of coronary heart disease with anxiety and depression in recent years, and to understand the current status of treatment, pathogenesis, dialectical classification and dialectical treatment.

ABSTRACT

Objective To explore the correlations between different traditional Chinese medicine (TCM) syndromes and the heart rate variability (HRV) accompanying patients with chest pain,and to provide a referen()for clinical syndrome differentiation in such patients.Methods A prospective study was conducted.()hundred and seventeen patients with chest pain admitted into Longhua Hospital Affiliated to Shanghai U()of TCM from January 2015 to October 2016 were assigned in a study object,and according to the diffe()TCM syndromes,they were divided into syndrome of qi deficiency with blood stasis,the blood sta()the deficiency of qi and yin,suppression of the chest yang,phlegm and blood stasis,qi-stagnan ()syndromes.In the same period,123 healthy people having undergone physical examination wer()control group.The time domain indexes of HRV were recorded by dynamic electrocardiogr()normal control group were compared to those of patients with different TCM syndrom()distribution in different TCM syndromes and various HRV time domain indexes we()cycle time domain indexes were as follows:the average standard deviation o()standard deviation of R-R interval (SDNN),24 hours standard deviation of()5 minutes (SDANN),24 hours the mean square root of difference values,()were observed in both groups.Results The chest pain patients a()accounting for the largest proportion (35 cases,29.9%),and ac()smallest proportion (5 cases,4.3%) in the study group;the nu()syndrome (17 cases vs.14 cases),blood stagnation of hea()(6 cases vs.2 cases) and qi-stagnation and blood stasis s()in men;the numbers of patients with deficiency of qi()(4 cases vs.2 cases) in men were more than thos()female patients were lower than those in maj()66.8 (33.4,33.4) vs.103.4 (39.7,135.4),124.7 (88.0,143.4) vs.167.0 (90.5,230.1),84.0 (22.5,132.6) vs.152.4 (31.4,240.0),all P < 0.05].The SDANN in patients with chest discomfort accompanied by any one of the above mentioned types of TCM syndrome was significantly lower than that in the control group,and its degree of descent was more remarkable in the patients with qi deficiency with blood stasis,the blood stagnation of heart and suppression of the chest yang syndromes (ms:74.86± 25.69,80.39± 20.53,70.97± 23.53 vs.131.30± 34.70,all P < 0.05);the SDNN of patients with deficiency of qi and yin syndrome was higher than that in the blood stagnation of heart syndrome,phlegm and blood stasis,and qi-stagnation and blood stasis syndrome significantly (ms:181.25 ± 65.20 vs.97.88± 23.61,84.28 ± 22.34,89.93 ± 8.43,all P < 0.05);the RMSSD of patients with deficiency of qi and yin syndrome was increased significantly compared with that in the healthy controls and in patients with the blood stagnation of heart syndrome (ms:91.94 ± 44.02 vs.28.00± 10.50,32.21 ± 18.25,both P < 0.05).Conclusions Patients with chest pain accompanied by different TCM syndrome types may develop obvious heart rate variability,and the descent of SDANN level was the most significant.The analysis of HRV changes in such patients has positive significance for their diagnosis and treatment.

ABSTRACT

Chest discomfort is a common challenge to clinicians; conditions vary from benign to life threatening. The differential diagnosis includes and overlaps cardiovascular, respiratory, gastrointestinal, neuromusculoskeletal & psychiatric problems, all requiring careful history and thorough investigations. However, a significant number of cases remain undiagnosed despite meticulous investigations and thus require empirical therapy (Fagring et al, 2005; Thomas, 2012). The present study was aimed to find the effect of one of the Proton-pump inhibitors (PPIs; Rabeprazole) on patients having non-traumatic, non severe chest discomfort, diagnosis of which could not be confirmed. Forty eight eligible patients were divided into 2 groups: PPI group and placebo group. Before and after 4 weeks of therapy, they were studied with visual analog scale for pain and its frequency, duration and intensity. Rabeprazole treated patients responded much better than the placebo group. Seventy five percent were pain free or had appreciable relief in pain intensity after 4 weeks. In contrast to Rabeprazole group, in placebo group none became pain free; 45 % had marginal relief while 55 % derived no benefit. Proton-pump inhibitors are of value in the treatment of non specific chest discomfort.

ABSTRACT

A Kampo formulation, Senkinho-Karo-to, is considered to be effective for chest pain or discomfort. However, there have been few case reports concerning this formulation. Here, we report four cases of patients suffering from chest pain or discomfort that were successfully treated with Senkinho-Karo-to. We compared the clinical features of these four cases where Senkinho-Karo-to was effective with those of four other cases where Senkinho-Karo-to was ineffective. The cases where Senkinho-Karo-to was effective had the following features: Deficiency, feeling of spasmodic uprising heat, shoulder stiffness, deficient pulse, impressions of teeth on the rim, palpitation of the abdominal aorta in the supra-umbilical region, reduced tension in the lower abdomen, tenderness in the para-umbilical region, and Ki-depression.
